Put your card in a safe place, since it may not work if you punch holes in it, bend it, or otherwise damage it.
If you are a new faculty or staff member, go to the Human Resources Department to have your personal and work related information added to the university's database. The IT department uses this information to validate that you are a university employee. If you are not in the system, we can’t give you an ID Card. Human Resources will give you a form to bring to the IT department. This form must be completed and signed by Human Resources Personnel Only. Bring this form to the IT Service Desk in the basement of the Blackmore Library to get your card.
If you are a contractor and you are either working on projects for the university or working for Parkhurst, Aetna or Conference Services, you must bring signed documentation from the hiring manager to the IT department. If you are a hiring manager, you may bring the employee with you to the IT department. Picture ID is mandatory (ex: drivers license, passport) for verification purposes. If you do not have signed documentation from the hiring manager and picture ID, we cannot issue an ID card to you.
