Special Circumstances

  • The purpose of the Special Circumstance form is to inform us of unusual or extraordinary information that could not included on your FAFSA. The Department of Education permits college financial aid offices to review, and possibly make adjustments to your FAFSA.

     
    Please contact the Financial Aid Office if, after reviewing the form below, you are unsure how to report your circumstance. 

    Requesting Special Circumstance Review

    All forms are in PDF format and require the free Adobe Acrobat reader.

     

    1. Print and complete a Verification Worksheet (please include all documents requested on worksheet)

       
    2. Download and complete the Special Circumstances Form (PDF). Please provide all information pertaining to your income and resources. 

       

    When the requested documentation is received, Capital will complete the Federal Verification review process and make any possible adjustments as a result of your circumstance. The student will receive a Financial Aid Award notification indicating the special circumstance review has been completed.
